Schacht:<\/b> Okay. Okay, what I want you to do is to describe the person, and uh, describe them the way they look, so that they can be recognized in a group of other people and be selected out. Okay. That means you\u2019ll have to describe their\u2013 their age<\/i>, their sex<\/i>, and their general appearance, if they\u2019re alert, if they\u2019re cooperative, if they\u2019re in acute distress or they\u2019re uh\u2013 (Tape edit)<\/p>\n

Woman:<\/b> (Unintelligible, low tone)<\/p>\n

Schacht:<\/b> \u2013that\u2019s right, if they\u2019re anxious, if they have any uh, problems at all that are uh, evident. If they come in in a wheelchair, then you describe that, for example. If they are on a stretcher, then you describe that. Okay, basically, you just describe their physical appearance. Okay?<\/p>\n

Then the uh, next thing you do is you take their vital signs, remembering that when you take the blood pressure, you occlude the radial pulse. And when you take the temperature<\/i>, you state whether it was taken orally, whether it was taken uh, in the axilla or taken rectally. But always state where<\/i> the temperature was taken from. Always state whether the pulse is\u2013 when you take the pulse, take the pulse for 15 seconds and then multiply times four to get the rate per minute, and uh, state whether the pulse is regular and uh, grading\u2013 use a grading system from one plus to four plus, the normal pulse being two plus.<\/p>\n

Woman:<\/b> (Unintelligible, low tone)<\/p>\n

Schacht: <\/b>(Unintelligible, low tone) Okay then, generally, the things that you\u2019re expected to know for the test are, I want a\u2013 we\u2019ll expect a general description of the person themself, their age, their sex, and any uh, characteristics which separate them from other people. (Tape edit) I uh, want you to describe how they present, whether\u2013 whether\u2013 (Tape edit) Okay, if the person presents on a stretcher, you\u2019re to describe that. If they\u2019re in acute distress, if they\u2019re cooperative, if they have a uh\u2013 (Pause) if they have anything about them which\u2013 which will distinguish them from other people, that\u2019s what you\u2019re supposed to describe. Then when you do the vital signs, I want you to take the pulse radially on both hands \u2013 take both radial pulses \u2013 and remember that a two-plus pulse is about normal. Three-plus is a very firm strong pulse, four-plus is very<\/i> strong, and one-plus is palpable<\/i>, but just barely so. Then it\u2013 when you take the blood pressure, be sure that you occlude the radial pulse. Go above that about ten millimeters on the scale, and then let down the blood pressure cuff until you hear the first sounds over the brachial artery.<\/p>\n

Woman:<\/b> Um-hmm [Yes]. (Tape edit)<\/p>\n

Schacht: <\/b>When you take the temperature, be sure that you record whether it was taken orally or axillary or rectally. And you should take the temperature orally, and you should get used to using a centigrade scale, so that 37 degrees is normal. (Tape edit) Remember that a slight variation in temperature is\u2013 is\u2013 is also normal. (Tape edit) When you take the respiratory rate, you\u2013 you take the person\u2019s pulse so that they\u2019re not anxious and that they\u2019re not concerned about uh\u2013 concerned and uh, you can get an accurate recording of their respiratory rate.<\/p>\n
