On December 5, 1978, Juanell Smart was interviewed at her residence, 2001 South Haster, Apartment 14A, Anaheim, California. She advised that she had resided at Jonestown, Guyana from July 18, 1978, to August 5, 1978. She had no information concerning guns, drugs, hit lists, contingency plans or any illegal activities concerning the People’s Temple. She was a member in Los Angeles, California from 1972 to 1975. The only weapons she ever saw were in 1974, when she saw one rifle and two pistols which two unknown members turned in at church.<\/p>\n
She was notified by the United States State Department <\/p>\n

that her following children were identified among the dead at Jonestown:<\/p>\n
Daughter, Pinetra [Tinetra] Fain, Negro female, date of birth November 8, 1958.<\/p>\n
Son, Alfred Smart, Negro male, date of birth June 3, 1960.<\/p>\n
The following two children are missing and presumed dead:<\/p>\n
Daughter, Teri Lynn Smart, Negro female, date of birth September 11, 1964, 4’10” in height, 75-80 pounds, small burn scar on right cheek; she has forwarded dental records to Dover Air Force Base.<\/p>\n
Son, Scott Cameron Smart, Negro male, date of birth June 11, 1973, 5’8″ in height, 120 pounds, numerous scars on both legs, mostly below knees, small scar outer corner of right eye. Recently had dental work performed at Georgetown, Guyana by Dr. Ng-A-Fook.<\/p>\n
In addition, Smart advised that the United States State Department advised that her mother, Enola Nelson, also known <\/p>\n

as Kay Nelson, date of birth November 9, 1920, had been identified as one [of] the dead in Jonestown along with her uncle, James McElvane.<\/p>\n
Armed and dangerous, suicidal tendancies [tendencies].<\/p>\n
