TO ENABLE DEPT TO COMPLETE SECTION REGARDING PERSONAL EFFECTS ON REPORT OF DEATH (SECTIONS 11 AND 12) INDICATE WHETHER ANY PERSONAL EFFECTS IDENTIFYABLE AS BELONGING TO THE PARTICULAR DECEDENT RECOVERED AND IF SO INCLUDE NAME OF PERSON OR AUTHORITY RESPONSIBLE FOR CUSTODY AND ACCOUNTING THEREFOR.
