In reviewing death certificates received from Guyanese authorities we noted one received relating to subject. Original of this death certificate among those 599 pouched to Department on December 30, 1979. Guyanese authorities being informed that [Simon] known to be survivor so that the records can be adjusted. Please do not repeat not prepare FS-192. Appreciate return original certificate this office for return Guyanese registrar.
