The next of kin should attach the name of the deceased person whether the person was male or female and date of birth as well as clearly identifying the name of next of kin and telephone number.
The personal identification done in Guyana while probably reliable is only the initial step in the verification process. Verification may be possible without these charts but it is desirable to have them on hand if necessary.
