This is a listing of the people who signed several forms granting limited power of attorney to Temple members for decisions relating to minor relatives.
The first form declares the person signing it to be the natural parent or legal guardian of the named minor, and the name(s) of the person appointed as “attorneys-in-fact.”
The second form grants consent for those attorneys to take the minor to Guyana.
The third form is a liability release.
The forms all appear in RYMUR 89-4286-B-1-a-, with the page number(s) indicated below. The names of the minors are in alphabetical order. The date of signature also appears next to the name.
