I – Nearly Blind – leave things pt is familiar w/ in some place. When some thing new arrives do explain thoroughly. – Pt can be sulf sufficient long time – [Illegible]

II – Watch weight not over 160 – Limit food intake. Keep wt below 160 for pts health sake – Will be able to do self help if not too heavy – Monthly

III – Watch fluid intake and output – offer fluids at least 5x daily – To help maintain fluid balance – Daily

IV – Watch for edema in feet and legs – Have proper fitted shoes No elastic in hose. Nothing to restrict circulation – No edema Good circulation – Daily

V – Personal appearance – Nearly blind need help is keeping + staying neat. Help to get dressed w/ [bath?], hair do’s, good shoes etc – To be well groomed Have a sense of security – Daily

  2. Clean baseboards
  3. Sweep floor (use knife to clean cracks)
  4. Mop floor
  5. Clean ceiling for cobwebs
  6. Make beds-move material from under beds
  7. Clean + organize clutter on shelves
  8. Empty pee buckets – sterilize with Detd
  9. Clean front porch – Knife for cracks
  10. Clean back porch
