1. AMONG GUYANESE DEATH CERTIFICATES SENT TO DEPARTMENT BY EMBASSY ON DEC. 30, 1978 WAS ONE FOR INFANT MONIQUE BACON, DPOB: 4/6/76 IN CALIFORNIA. MONIQUE WAS IDENTIFIED VISUALLY BY ONE OF PEOPLE’S TEMPLE SURVIVORS (SEE OUR TEL 3846 DATED 11/21/78). HER NAME HAS NOT SUBSEQUENTLY APPEARED ON ANY OF LISTS OF IDENTIFIED BODIES EMANATING FROM DOVER.
2. OUR FILES CONTAIN COPY IDENTIFYING DATA IN PHOTOGRAPH PAGE OF US PASSPORT H801169 AND MONIQUE’S INTERNATIONAL VACCINATION CARD WHICH SHOWS HOME ADDRESS TO BE 261 DIVISADERO ST., SAN FRANCISCO, CA., EMBASSY RECORDS OF SURVIVORS AND DECEASED SHOW NO OTHER PERSON WITH SURNAME OF BACON. IT SEEMS REASONABLE TO ASSUME THAT MONIQUE WAS IN JONESTOWN WITH A RELATIVE OR GUARDIAN, LEGAL OR INFORMAL, AND THAT IN FACT SHE DIED DURING THE MASSACRE. THE EMBASSY, THEREFORE, PROPOSES TO CONTINUE TO HANDLE THIS AS A DEATH CASE AND WILL SIGN THE FS-192 WHEN RECEIVED FROM DEPARTMENT IF NO OBJECTION IS PERCEIVED.
3. ANY DATA AVAILABLE TO THE DEPARTMENT REGARDING NEXT OF KIN WOULD BE VERY USEFUL TO EMBASSY.
