My name is Ann Bumbury. I am not married. I am the mother of the baby boy Travis Lang born to me December 1, 1977. I am not able to raise my said child, and wish to leave it with a foster parent to be, Peoples Temple agricultural project permanently. Any such person has my consent to do all things necessary to raise my baby. If a formal adoption is desired by such foster parents, I hereby give my full consent to any such adoption proceeding.
