The following investigation at Los Angeles was conducted on January 12, 1979 in an effort to obtain medical records for Ronnie Dennis, Negro male born December 20, 1961:

[Name deleted], Administrator, Department of Health Services, Hubert H. Humphrey Comprehensive Health Center, 5850 South Main Street, Los Angeles, California, advised the Health Center had no record for a Ronnie Dennis, except for a Martin Luther King, Jr., hospital number 17-04-94.

Services, Martin Luther King, Jr., General Hospital, 12021 South Wilmington Avenue, Los Angeles, California, 90059, advised Ronnie Dennis, Martin Luther King number 17-04-94, was admitted to Martin Luther King, Jr., hospital on February 24, 1976 and discharged February 25, 1976, for an eye problem. He was an outpatient on November 18, 1975, when an x-ray was taken of both his knees. [Name deleted] advised the knee x-ray was the only x-ray in his file. No record was indicated for dental care but there was a Florence Firestone Health Clinic number 692-68-0150. [Name deleted] advised he could not release the x-ray of the patient’s knees without a written request from the proper authorities at Dover Air Force Base. This request should be directed to [Name deleted], Administrator, Martin Luther King, Jr., General Hospital, 12021 South Wilmington Avenue, Los Angeles, California, 90059.

[Name deleted], Registrar, Los Angeles County Department of Health Services, Health Center, 1522 East 102nd Street, Los Angeles, reviewed Florence Firestone Health Clinic file number 692-68-0150 for Ronnie Dennis. He advised Dennis

was treated for a dog bite on the upper left arm on June 26, 1972. No stitches indicated. There were no x-rays in the file. There was a written record of dental work performed at the Florence Firestone Health Center.

All fillings were silver.

November 16, 1970, occlusal filling on first molar, upper left. November 30, 1970, occlusal filling for first molar, lower right. December 21, 1970, occlusal and buccal first molar, lower left. January 25, 1971, upper right first molar, occlusal filling, distal lingual gingival.
