On 12/5/78 Colonel B. L. Ramsey, U.S. Army representative to State Department, who has been working with the Guyana Working Group at State advised FBI HQ that he had received information from the National Personnel Records Center (NPRC), St. Louis, Missouri, that a person identifying himself as Captain [name deleted], U.S. Marine Corps (USMC), had telephoned NPRC that day asking the location of an individual who reportedly could identify some of the victims of the Jonestown “incident” in Guyana . [Name deleted] left a phone number and address, but when NPRC telephoned him he was not known to anyone at that number, which turned out to be a Pentagon phone number.

On 12/6/78, [Name deleted], NPRC, advised FBI HQ that on 12/5/78 someone calling himself Captain [Name deleted], USMC, who said he was working on the “Guyana incident” at the State Department, telephoned NPRC to ask for the last known location of [Name deleted], who reportedly had worked for the Department of Defense (DOD) at Cameron Station in 1977, and could identify victims from Jonestown. [Name deleted] gave his Washington, D.C., phone number as 693-0692 and asked to be called back as soon as possible. Further, he gave his address as “1220 12th Street, 1001, Washington, D.C. 20005.” [Name deleted] related that NPRC determined only one [Name deleted] was born [date deleted], possessed Social Security number [deleted] and his personnel file had been sent by NPRC in 1973 to the National Foundation on the Arts and Humanities, 806 15th Street N.W., Washington, D.C.

When NPRC attempted to reach [Name deleted] at 693-0692, which is a Pentagon number, it was ascertained that no one there knew [Name deleted]. NPRC also checked with the information officer, USMC, Pentagon, and there was no listing for [Name deleted] in USMC records.

WFO should attempt to locate [Name deleted] at the above address for additional information in this matter, and also attempt to locate [Name deleted] through contact at the National Foundation on the Arts and Humanities, in an effort to determine whether he in fact is able to identify victims.
