Casualty message
1. It is with profound regret that we must inform you of the death on September 9, 1978 of Vivian Ida Anderson. The People’s Temple agricultural project at Jonestown, Guyana informed the American Embassy in Georgetown on September 29, 1978 of her death of cardiac arrest.
2. Representatives of the People’s Temple have turned over to the Embassy American passport #G2235202 which was issued to Vivian Ida Anderson on October 13, 1976 in Los Angeles. This passport contained your name and address as next of kin and was received by the Embassy only two days ago.
3. The People’s Temple organization has informed us that Ms. Anderson was buried in their cemetery at Jonestown in accordance with her wishes.
4. The consular section of the Embassy will take charge of all

personal effects and will write for your instructions as to their disposition.
5. Upon completion of all formalities a Foreign Service Report of Death prepared by this office will be forwarded to you. This document is based upon Guyanese death certificate and can be used in U.S. courts to settle estate matters.
6. If necessary, you may contact the consular section by phoning 62687 in Georgetown, Guyana between the hours of 4:00 A.M. and 7:00 A.M. and 9:00 A.M. and noon California time.
7. May I extend my deepest sympathy to you in your bereavement.
