1. [Freddie L. Lewis] contacted Department concerning W/W of [next four lines give names and dates of birth of Fred Lewis’ wife and seven children, all named above]. Reportedly they left US for People’s Temple, Jonestown, in August, without [Mr. Lewis’] knowledge. He has not heard from them since departure.
2. On next visit to Jonestown, please contact Mrs. Lewis and children. Please provide W/W Report be a direct relay to [information identifying Freddie L. Lewis, including address at 366 Scott Street, San Francisco, California 94117] .
