J-3 • Summaries of folders

J-3-a begins with a note about Leta Murphy experiencing renal failure, being sent to G.T. (Georgetown), and then being sent back to Jonestown where she died. This is followed by four pages labeled Medical Dept. Listed By Depts. These pages list the offices within the medical department with typewritten names of the members working in them. Next to the typewritten names are handwritten notes about the person’s duties. The fourth page ends with a written note stating, “I don’t know how we change the structure of the staff without losing out on important things that have to be done. Phyllis” Next is a tag labeled Drug Inventory Records followed by an envelope and its contents to Vernon Gosney from Western Medical Enterprises, Inc. It includes a bill for several Valium and Vicon C prescriptions and a handwritten note suggesting asking Gene Chaikin if the bill had to be paid. The remaining pages are almost entirely handwritten notes of prescription drug inventory counts. Many have been scribbled over and are barely legible, if not entirely blacked out. Several of the dates provided are mid-October 1978 and match some of the latest provided inventory count dates found in J-1. There are also some children’s art, a blank authorization form by Corinne (Rennie) Kice, and four dispensary record pages matching the style found in J-1. One is blank, one is illegible to me at this time, one is for Avomine (Promethazine theoclate, an antihistamine), and one for Nembutal (Pentobarbital, 100 mg).

J-3-b contains more handwritten prescription drug inventory counts as well as inventory lists dated September 7th, 1978. These inventories are categorized by container and person, possibly the person that brought the container to Jonestown. The items are varied such as hand saws, cassette players, feminine hygiene products, socks, and medical equipment. There are also scans of requests for pricing information for medical equipment such as a half-gallon of acetone, diabetic items, and laboratory equipment and other orders or requests from the medical department.

J-3-c contains 4 pages of inventories, one each numbered in order of appearance: Inventory Trunk #215, Inventory Trunk #217, Inventory Trunk #216, and inventory for box #214. All four pages are signed by Steven Daniel and are stamped on the middle left of the page, SHIPPED OCT 23 1978. The last page includes a handwritten note saying “To Georgetown Coordinator Tim Carter.”

J-3-d is two pages titled Suggested Dealer Service Kit for Kodak X-Omar Processor, Model M6. (This is a film processor designed to rapidly process medical x-ray film.) It includes the part name, number, and suggested quantity. Some quantities are crossed out with lower numbers handwritten next to the crossed out number.

J-3-e comprises three pages of numbered instructions from Jim Jones to the Medical Department, numbering 67 instructions in total.

J-3-f begins with Evaluations of the Medical Department, handwritten and dated 30/9/78. Included is a single page in different handwriting with the same date labeled “Evaluations from SCU.” Following is another evaluation of the medical department dated 20/10/78. Although it is itself undated, J-3-f-3c mentions Lynetta Jones, despite all other dates in the file occurring after her death in Jonestown.

J-3-g is an assorted collection of medical records for Marceline Jones, mostly from the office of Dr. Carlton B. Goodlet during Spring 1977. Included is a note to Marceline’s employer from Dr. Goodlet regarding his suggestion to take medical leave as well as payroll time sheet requesting sick leave and vacation time.
